Effect of Yoga Basti with Tila Taila and Dashamoola Kwatha in the Management of Gridhrasi (Sciatica): A Case Report

Gridhrasi is described in Ayurveda under Vata Nanatmaja Vyadhi and is characterized predominantly by pain radiating from the low back and hip region towards the lower limb, along with stiffness, pricking sensation and restricted movements. Clinically, the presentation of Gridhrasi closely resembles sciatica, in which pain follows the distribution of the sciatic nerve. Basti Karma is considered an important therapeutic modality in the management of Vata disorders.
A 44-year-old male patient presented with complaints of pain in the lower back for approximately 7–8 months. The pain was predominantly on the right side and radiated to the right lower limb. The patient experienced difficulty in routine activities associated with the pain. On clinical examination, Straight Leg Raising (SLR) test was positive on the right side at 30°. No MRI investigation was available; therefore, the diagnosis was made on the basis of clinical presentation and examination findings.
From an Ayurvedic perspective, the clinical presentation was considered consistent with Gridhrasi, predominantly Vata involvement. The patient was treated with a 16-day Yoga Basti protocol consisting of Tila Taila Anuvasana Basti and Dashamoola Kwatha-based Niruha Basti, along with oral Shamana Aushadhi comprising Trayodashanga Guggulu, Dashamoola Kwatha, Ekangaveera Rasa and Hingvashtaka Churna.
The patient showed marked improvement in low-back pain, radiating pain and functional mobility following completion of the treatment course. The SLR test, which was positive at 30° before treatment, showed substantial improvement after treatment. The patient also reported improvement in walking, sitting, bending and routine activities.
